Description

Project H.E.A.L. (Health, Education, Access, Link) is an outreach program of CentraCare Family Health Center. Project H.E.A.L. offers free health screenings and basic care at various Central Minnesota locations for those who have little or no insurance.

The goal of Project H.E.A.L. is to overcome the barriers to health care for the under-served population of Central Minnesota — barriers of transportation, inadequate or lack of insurance, financial resources for co-payments and medications, language and culture.

Locations

  • St. Cloud
    • Place of Hope - 320-203-7881, 2nd Wednesday of each month at 5 p.m.
    • Salvation Army Shelter - 320-252-2229, 1st & 3rd Thursdays at 5:30 p.m.
  • Long Prairie
    • Reynolds Church - 3rd Saturday at 9 a.m.
    • West Campus - 1st Monday at 6:00 p.m. 
  • Monticello
    • Monticello High School - 320-240-3116, 1st Thursday of each month at 5 p.m.
  • Big Lake
    • Big Lake Community Food Shelf - 763-263-2432, 1st Monday of each month at 5 p.m.
  • Sauk Centre
    • River of Life Church - 320-352-2315, 1st Saturday of each month at 9 a.m.

Partners

The partners for Project H.E.A.L. include local congregations, Place of Hope, Catholic Charities, Salvation Army, Central Minnesota Parish Nurse Program, CentraCare Family Health Center, St. Cloud Hospital, St. Cloud Legal Services, Stearns-Benton Medical Society.
